A custom icon does not appear after you add a custom icon to a custom entity when you view the entity list in Microsoft Dynamics CRM 3.0

When you view the entity list in Microsoft Dynamics CRM 3.0, a custom icon does not appear after you add a custom icon to a custom entity. Additionally, the default icon does not appear. Instead, the Windows Internet Explorer image placeholder appears.
